BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2016218848 DESCRIPTION A girl aged 16 years presented with sudden loss of vision in the left eye (LE) for 1 day. Visual acuity was 6/6 in the right eye (RE) and perception of light in LE. LE had relative afferent pupillary defect. RE fundus had cotton wool spots (figure 1A). LE fundus showed combined central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO) (figure 1B). Her history of alopecia, fever and arthritis for 6 months, bilateral cervical adenopathy, hard palate ulcers, leucopenia (2300/mm), lymphopenia (621/mm), positive antinuclear antibody and undetectable serum C4 led us to diagnose systemic lupus erythematosus (SLE). Lupus anticoagulant, anticardiolipin (IgM, IgG) and anti-β2 glycoprotein1 (IgM, IgG) antibodies were negative. She was treated with highdose corticosteroids and anticoagulant therapy. On follow-up, she was also started on immunosuppressive agent methotrexate as a part of steroid-sparing therapy. Two weeks later, she received intense pan-retinal photocoagulation in LE to prevent neovascular glaucoma. Despite these efforts, she developed vitreous haemorrhage in LE on follow-up. Combined CRAO and CRVO though rare is a severe ocular complication of SLE resulting in irreversible loss of vision. Prompt diagnosis and early management in such a case is important to treat underlying systemic vasculitis and to prevent occurrence of similar episode in the other eye. 2

Antiphospholipid syndrome in lupus retinopathy

Systemic lupus erythematosus (SLE) is a chronic, multisystem, autoimmune disease that can affect any part of the human body including the eyes. Common blinding ocular manifestations include central retinal artery occlusion (CRAO), central retinal vein occlusion (CRVO), severe vaso-occlusive retinopathy, and optic nerve involvement. Antiphospholipid syndrome (APS) in lupus is usually associated ...

Combined Central Retinal Vein and Branch Retinal Artery Occlusion Post Intense Physical Activity

We report a case of combined central retinal vein occlusion and branch retinal artery occlusion. A previously healthy 47-year-old male presented with decreased vision in the right eye after completing a half marathon. A fundus exam and retinal imaging revealed a combined central retinal vein and branch retinal artery occlusion. In the present report, we review the literature and discuss the pos...

Detection of underdiagnosed concurrent branch retinal artery occlusion in a patient with central retinal vein occlusion using spectral domain optical coherence tomography

BACKGROUND Combined branch retinal artery and central retinal vein occlusion is a rare condition that has been infrequently reported. This case report, aside from reporting the above-mentioned condition, highlights the importance of performing spectral domain optical coherence tomography in establishing a complete diagnosis, especially in uncertain and complicated cases. We also present spectra...

Optical Coherence Tomography Angiography of Combined Central Retinal Artery and Vein Occlusion

Optical coherence tomography angiography (OCTA) is a new, noninvasive technology that enables detailed evaluation of flow in the retinal and choroidal vasculature. The authors believe this to be the first report to describe the optical coherence tomography angiography findings associated with combined central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO).

Combined Central Retinal Artery and Vein Occlusion Associated with Factor V Leiden Mutation and Treated with Hyperbaric Oxygen.

BACKGROUND Combined central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO) is an uncommon retinal vascular disease which causes sudden visual acuity loss and is associated with poor prognosis and the development of severe complications. We report a very rare case of combined CRAO and CRVO in a patient with factor V Leiden (FVL) mutation (only 3 cases published). To ou...
